At FoundationMED, each of our physicians partners with a nationally board certified health and wellness coach to ensure that you have all the support you need to get the results you seek
Chances are you already have some idea what lifestyle changes you need to make. After meeting with your physician you’ll have an even greater appreciation for how those lifestyle changes will determine the pace and success of your healing journey.
But changing long-held habits and behaviors and shifting mindset is hard.  Your FoundationMED health coach knows how to support these changes within the framework of your physician-directed treatment plan.
Coaching bridges the gap between knowing what you need to do, and getting it done— in a way that lasts. You are the expert on the circumstances and obstacles that can get in the way of your success.  Your health coach is the expert on how to help you break through those challenges.  They provide support, accountability and resources as you progress towards optimal health and wellness.
Additionally, your FoundationMED health coach is also your “patient coordinator”
They are the connection between you and your FoundationMED physician for questions directly related to your treatment plan.  At the end of each office day, they confer with the physician and will respond to you in a timely manner.  This unique combination of health coach as change agent AND communication resource results in the “5 star experience” we strive for with every patient interaction.
Kelly O'Brien, NBC-HWC Kelly Day O’Brien is a Duke Integrative Medicine trained Integrative Health Coach and has over 25 years of healthcare-related experience. As a FoundationMED Health Coach, Kelly provides resources, support and accountability as you strive to make healthy changes. She has extensive experience with the diet and lifestyle recommendations that serve as the foundation for your treatment plan. Guided by her coaching expertise, you’ll set a plan in motion to achieve your vision of optimal health. Through a collaborative partnership, you’ll identify obstacles to change and create strategies for sustainable health and wellness goals.
