Health coaching bridges the gap between knowing what you need to do (navigating your treatment plan) and actually doing it (making the necessary lifestyle and habit changes).

We understand that this can feel challenging, frustrating or even insurmountable—especially if this isn’t the first time you’ve tried to make these changes!  

Your FoundationMED Health Coach has a genuine concern for your thoughts and feelings.  We are trained to be compassionate listeners, and will defer to your expertise when it comes to your life experiences and unique challenges.  

 

We do support your exploration of insights using motivational interviewing skills and neutral, non-judgmental curiosity.  

We do empower you to actively engage in your treatment plan, and encourage discovery and self-advocacy.

We do guide your understanding of your underlying conditions and symptoms, how they relate to your treatment plan and the lifestyle recommendations of your FoundationMED physician.

We do provide expertise and information about functional wellness as you discern the strategies, tactics and tools that you’ll need to realize your vision of optimal wellness.

 

We don’t provide any diagnosis, create treatment plans or give nutritional advice outside of the recommendations of your physician-directed plan.  

We don’t interpret or order labs, recommend supplements or provide counseling or psychotherapy.
